Restore First Health is seeking a highly skilled Wound Care Coordinator to join our team in Plymouth Meeting, PA. As a key member of our clinical staff, you will play a vital role in ensuring the delivery of high-quality patient care and implementing our Mobile Advanced Wound Therapy Program's policies and procedures.
Key Responsibilities- Perform intake on all referrals and schedule consultations and assessments for patients with pressure ulcers and chronic wounds.
- Review patient charts, ensure accurate documentation, and implement treatment plans in collaboration with our medical team.
- Investigate adverse events related to wounds and participate in clinical outcome monitoring and performance improvement initiatives.
- Work closely with our Authorization Specialist to confirm coverage benefits and prior authorizations.
- Develop and implement standards of care, policies, and procedures in collaboration with our Medical Director and leadership team.
- Nursing degree from an accredited college or university or equivalent experience.
- Active RN or LPN license to practice in Pennsylvania.
- Excellent organizational, prioritizing, and communication skills.
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks.
