Job Description: Clinical Care Coordinator

We are seeking a skilled Clinical Care Coordinator to join our team at South Central Health System. As a key member of our healthcare team, you will be responsible for coordinating patient care services, ensuring seamless communication, and promoting positive health outcomes.

As a Clinical Care Coordinator, you will work closely with our clinical staff to coordinate patient care services, communicate with patients and families, and ensure efficient workflows.

Key Responsibilities:
- Coordinate patient care services, including scheduling appointments, lab tests, and radiology procedures
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination
- Maintain accurate and up-to-date patient records, ensuring compliance with regulatory requirements
- Assist with quality improvement initiatives to enhance patient care and outcomes
- Participate in interdepartmental teams to promote collaboration and optimize patient care

Requirements:
- High school diploma or equivalent required; associate's degree or higher preferred
- Previous experience in a healthcare setting preferred
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Basic computer skills and knowledge of electronic medical records software

Salary: $42,000 - $58,000 per year, depending on experience
