Job Title: Mental Health Specialist

Horizon House is a leading provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. We are seeking an On-Call/PRN Mental Health Specialist to work in our Homeless Services Division.

Job Summary:

The Mental Health Specialist will provide community-based supports to individuals in compliance with the Agency service contract and policies and procedures. Approximately 75% of work is done in the community. The selected candidate will do home-based visitations and interact with members of the multidisciplinary team to ensure that participants receive individualized services designed to meet their needs and to help them successfully maintain housing and accomplish their recovery goals.

Responsibilities:
  • Provide service coordination (case management for an assigned group of participants) including coordinating and monitoring the activities of the individual treatment team.
  • Assume responsibility for developing, writing, implementing, evaluating and revising overall treatment goals and plans in collaboration with the participant and the ITT.
  • Provide individual supportive therapy, psychotherapy, and symptom management, ensuring immediate changes are made in the treatment plans as participants' needs change.
  • Educate and support participants' families, and advocate for participants' rights and preferences.
  • Perform shift management in coordination with other ACT shift managers according to established policies and procedures.
  • Provide individual supportive therapy (e.g., problem solving, role-playing, modeling and support), social skill development, and assertiveness training to increase participants' social and interpersonal activities in community settings.
  • Provide services in compliance with contract and Agency regulations.
  • Conduct comprehensive assessment of psychiatric history (e.g. onset, course and effect of illness, past treatment and responses, and risk behaviors), mental status, and diagnosis; physical health and dental health; use of drugs or alcohol; education and employment, social development and functioning; activities of daily living (e.g., self-care, living situation, nutrition, money management); and family structure and relationships.
  • Participate in the resolution of crisis situations; assist with identifying key individuals and resources that can provide direct support and/or immediate intervention during a crisis situation.
Requirements:
  • Masters degree in Social Sciences, Healthcare or related field and Post-secondary school education and training is required, including internships and other supervised practical experiences in a clinical or rehabilitation setting with persons with severe and persistent mental illness.
  • Must be able to communicate both verbally and in written form with every level of the behavioral health system and maintain effective professional relationships with associates in the Agency/community at large.
  • Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illnesses and respect for participant rights and personal preference in treatment are essential.
  • Ability to work independently and as part of focused team.
  • Working knowledge of Outlook, MS Word and Excel.
  • Ability to maintain daily productivity standards.
  • Fee-for-service and electronic health record experience strongly preferred.
  • Must possess a valid driver's license with an acceptable driving record.
