Job Overview:

The Public Health Management Corporation (PHMC) is seeking a skilled Mental Health Care Specialist to join our team. As a key member of our healthcare team, you will provide high-quality mental health services to adults in community-based nurse-managed health centers, serving diverse and underserved populations.

Responsibilities:

  • Conduct comprehensive bio-psychosocial evaluations for adults at health centers and outreach sites.
  • Provide medication management services using evidence-based guidelines at health centers and outreach sites.
  • Develop and lead group education programs for homeless individuals, promoting healthy behaviors and self-management techniques.
  • Refer clients to psychiatric and emergency departments, substance abuse treatment providers, and other relevant services as needed.
  • Counsel individuals with mental illness and substance abuse disorders on effective management strategies.
  • Implement evidence-based practices, including cognitive behavioral therapy and self-management techniques.
  • Train shelter staff on behavioral health management and other health issues, ensuring comprehensive care.
  • Participate in advocacy and coordination committees addressing homelessness-related issues.
  • Provide consultation services on behavioral health to Health Care for the Homeless management and clinical staff.
  • Train community and academic healthcare professionals to deliver culturally competent behavioral health care to homeless individuals.
  • Notify the Infection Control Coordinator immediately of suspected or confirmed communicable diseases that can be transmitted through routine shelter contact.
  • Provide psychiatric treatment or consultation services to assigned clients and families, managing an assigned caseload and ensuring timely documentation.
  • Ensure psychiatric evaluations and medication management documentation meet stakeholder, payor, and regulatory expectations.
  • Utilize evidence-based practices for individual clients and program philosophy.
  • Ensure clients receive psychiatric services based on need and contractual agreements, integrating services into overall treatment plans.
  • Prescribe buprenorphine-naloxone/buprenorphine to patients enrolled in the Medication-Assisted Treatment (MAT) program and in compliance with treatment contracts.
  • Perform other duties as assigned.

Requirements:

  • Experience in behavioral health, primary care, and/or community health preferred.
  • Excellent written and verbal communication skills.
  • Experience working with homeless individuals and racial minorities preferred.
  • Preferred credentialed with insurance companies.
  • Bilingual preferred.

Education and Licenses:

  • Education: Master of Science in Nursing (MSN).
  • Licenses: Pennsylvania Registered Nurse (RN) and Certified Registered Nurse Practitioner (CRNP) in psychiatric mental health.
  • Certification by the American Nurses Association (ANA) or equivalent preferred.
