Responsibilities:

  • Personal Care: Assist patients with daily living activities, such as bathing, dressing, and grooming.
  • Assist with Medication Management: Help patients manage their medication regimen, including administering medications as directed by the patient's physician.
  • Household Management: Assist with light housekeeping, laundry, and meal preparation.
  • Companionship: Provide emotional support and companionship to patients, helping to alleviate feelings of loneliness and isolation.
  • Communication: Maintain open and effective communication with patients, families, and healthcare team members to ensure seamless care coordination.
  • Documentation: Accurately document patient care activities, including progress notes and medication administration records.
  • Teamwork: Collaborate with the healthcare team to develop and implement individualized care plans that meet the unique needs of each patient.

Requirements:

  • Education: High school diploma or equivalent required.
  • Certification: Active and verifiable State Tested Nursing Assistant (STNA) or Certified Nursing Assistant (CNA) certification in applicable state.
  • Experience: 1+ year of experience in a healthcare setting, preferably in home health care.
  • Skills: Excellent communication and interpersonal skills, ability to work independently, and strong attention to detail.
