Job Title: Home Health Aide

Job Summary:

Community Based Care is seeking a compassionate and dedicated Home Health Aide to join our team. As a Home Health Aide, you will play a vital role in enhancing the lives of our patients by providing assistance with daily living activities and ensuring their comfort and well-being.

Responsibilities:

  • Provide personal care services to patients, including bathing, dressing, and grooming.
  • Assist patients with meal preparation and feeding.
  • Perform light housekeeping duties, such as laundry and cleaning.
  • Monitor and report any changes in patient condition or needs to the Registered Nurse.
  • Document care provided and complete required forms for patient records.
  • Communicate effectively with patients, families, and healthcare team members.
  • Maintain patient confidentiality and adhere to infection control policies.
  • Participate in required in-service programs and identify learning needs to the agency supervisor.

Qualifications:

  • Meet state training and certification requirements for Home Health Aides.
  • Active and verifiable State Tested Nursing Assistant (STNA) or Certified Nursing Assistant (CNA) certification.
  • Ability to read, write, and follow a written Plan of Care.
  • Good interpersonal skills and a passion for caring for others.
  • Current driver's license and reliable transportation.
