Job Summary

Community Based Care is seeking a compassionate and dedicated Home Health Care Professional to join our team. As a Home Health Care Professional, you will play a vital role in enhancing the lives of our patients by providing assistance with daily living activities and ensuring their comfort and well-being.

Key Responsibilities

  • Provide personal care services to patients, including bathing, dressing, and grooming, under the direction and supervision of a Registered Nurse.
  • Assist patients with meal preparation and feeding, as needed.
  • Perform light housekeeping duties, such as laundry and cleaning.
  • Document care provided and complete required forms for patient records.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care delivery.
  • Participate in required in-service programs to maintain compliance and stay up-to-date on industry best practices.

Requirements

  • Meet the training and/or certification requirements of the state in which services are being provided.
  • Hold an active and verifiable State Tested Nursing Assistant (STNA) or Certified Nursing Assistant (CNA) certification, or have 1 year of supervised, verifiable experience within the last 3 years.
  • Be at least 18 years of age.
  • Demonstrate the ability to read, write, and follow a written Plan of Care.
  • Exhibit good interpersonal skills and a passion for caring for those in need.
  • Hold a current driver's license and good driving record, or have reliable transportation.
