We are seeking a skilled RN Care Manager to join our team at Providence Medical Group. As a key member of our Elder at Home care team, you will provide high-quality, patient-centered care to our most medically fragile patients and their families.
Key Responsibilities- Coordinate care for patients with complex medical needs, ensuring seamless transitions between healthcare settings.
- Develop and implement personalized care plans, incorporating patient-centered interventions and engagement strategies.
- Provide education and support to patients and their families, empowering them to manage their care effectively.
- Collaborate with our team of healthcare professionals to ensure comprehensive, coordinated care.
- Monitor patient outcomes, identifying opportunities for improvement and implementing changes as needed.
- Oregon Registered Nurse License upon hire.
- 2 years of clinical medical-surgical, acute care, or long-term care experience, including 1 year in community-based/ambulatory care and 1 year in care of the frail elderly.
- Bachelor's Degree in Nursing preferred.
