We are seeking a skilled and compassionate RN Care Manager to join our team at Providence Medical Group. As a Care Coordinator, you will play a vital role in providing high-quality, patient-centered care to our most medically fragile patients in the comfort of their own homes.
Key Responsibilities- Provide comprehensive care coordination services to patients with complex medical needs, including medication management, wound care, and disease management.
- Collaborate with our team of healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.
- Conduct regular home visits to assess patient needs, provide education and support, and coordinate with other healthcare providers as needed.
- Work closely with patients and their families to promote self-management and independence in the home and community setting.
- Participate in multidisciplinary teams to develop and implement care plans, and to share best practices and knowledge.
- Oregon Registered Nurse License upon hire.
- 2 years Clinical medical-surgical, acute care or long-term care experience that also includes:
- 1 year in community-based/ambulatory care.
- 1 year in care of the frail elderly.
- Bachelor's Degree in Nursing.
- Home health, hospice, palliative care, managed care, or skilled nursing facility background.
- 1 year Care management of diverse patient populations.
