RN Care Manager

As a RN Care Manager with Providence, you will play a vital role in providing high-quality, patient-centered care to our most vulnerable patients. You will work closely with our interdisciplinary team to coordinate care, manage transitions, and promote self-management in the home/community setting.
Key Responsibilities- Provide care coordination services to patients with complex medical needs
- Manage transitions of care to ensure seamless handovers between healthcare providers
- Develop and implement individualized care plans to promote patient self-management
- Collaborate with healthcare providers to ensure timely and effective care
- Monitor patient outcomes and adjust care plans as needed
- Current Oregon Registered Nurse License
- 2 years of clinical medical-surgical, acute care, or long-term care experience
- 1 year of community-based/ambulatory care experience
- 1 year of care of the frail elderly experience
- Bachelor's Degree in Nursing
- Home health, hospice, palliative care, managed care, or skilled nursing facility background
- 1 year of care management of diverse patient populations
