We are seeking a skilled and compassionate Registered Nurse to join our Elder at Home care team as a RN-Care Manager. In this role, you will provide customized care coordination services and management of transitions of care for our most medically fragile patients and their families/caregivers.
Key Responsibilities- Provide across-the-continuum care coordination services to patients and their families/caregivers
- Manage transitions of care to ensure seamless and effective care delivery
- Develop and implement patient-centered interventions and engagement strategies to promote self-management in the home/community setting
- Collaborate with our provider group of eight physicians to provide urgent visits and assessments in the home
- Work closely with patients, families, and caregivers to understand their needs and preferences
- Oregon Registered Nurse License upon hire
- 2 years of clinical medical-surgical, acute care, or long-term care experience, including 1 year in community-based/ambulatory care and 1 year in care of the frail elderly
- Preferred qualifications include a Bachelor's Degree in Nursing, home health, hospice, palliative care, managed care, or skilled nursing facility background, and 1 year of care management experience with diverse patient populations
