About the Role

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Special Service for Groups, Inc - HOPICS. This is a full-time position that requires excellent communication and interpersonal skills, with a strong focus on providing exceptional support to our clients.

Key Responsibilities
  • Provide client care and service coordination under the direction of the Pathway Home Program Manager/Supervisor
  • Assist clients with daily tasks and provide linkages to necessary services
  • Meet with management staff regularly to discuss findings and develop individualized housing service plans
  • Work collaboratively with Program Managers, Supervisors, and Direct Service Staff to ensure seamless delivery of services
  • Perform administrative tasks, including maintaining client files and records in compliance with HIPAA and 42 CFR Part 2
Requirements
  • Associate degree in systems, social services, or a related field, with a minimum of 1-2 years of job-related experience
  • High proficiency in HMIS, Microsoft Word, Excel, Access, and other database programs
  • Ability to work with clients from diverse backgrounds and communicate effectively both verbally and in writing
  • A valid California Driver's License and proof of insurance are required
  • Verification of Employment Eligibility and Background Check are also required
Salary and Benefits

The estimated salary for this position is $45,000 - $55,000 per year, depending on experience. Our comprehensive benefits package includes health, dental, and vision insurance, as well as paid time off and opportunities for professional development.
