Job Overview

We are seeking a skilled Clinical Care Coordinator to join our team at Equanimity Home Health Services. This role plays a critical part in coordinating and managing patient care to ensure optimal health outcomes.

In this position, you will work closely with patients, healthcare providers, and community resources to develop and implement individualized care plans. You will assess patient needs, facilitate access to services, and provide education and support throughout the treatment process.

Key Responsibilities
  • Coordinate home care services for patients and assist with establishing the plan of care in coordination with the physician and the DON.
  • Maintain comprehensive working knowledge in the field and share information with appropriate organization personnel.
  • Work with Clinical Supervisors/Nursing Supervisors and staff clinicians to gather, collate, and report patient care summary information.
Requirements
  • MUST have experience checking and understanding OASIS-E to clarify proper data entry in timely manner.
  • MUST have experience reading and understanding new physician orders and generating new home health orders in a timely manner to be sent to the physicians.
  • Maintain comprehensive working knowledge of contractual relationships and ensure that patients are admitted and services are delivered in accordance with contract provisions.
