Salary Information

The estimated annual salary for this position is $48,595 - $60,692, based on a 24 pay period per year schedule with an hourly rate of $23.28 - $28.28.

Job Description

We are seeking a qualified Care Coordinator to join our Intensive Case Management Services (ICMS) Program. As a Care Coordinator, you will work closely with clients to develop personalized treatment plans that address their unique needs and goals.

Key Responsibilities

">
  • Case Management: Provide intensive case management services to clients, including independent living skills, housing stabilization, money management, community integration, employment linkage, benefits establishment, and linkage to community providers for substance use, primary, and mental health care.
  • Treatment Planning: Collaborate with clients to develop individualized treatment plans that address short- and long-term goals, and provide ongoing case management support to assess plan progress and ensure goals are met or adjusted as necessary.
  • Documentation: Maintain accurate and timely documentation of client interactions, progress, and outcomes, in accordance with program contract and PATH policies.

Requirements and Qualifications

  • Experience: Minimum 2 years of experience in healthcare, case management, homeless services, or working with vulnerable populations.
  • Educational Background: High school diploma or equivalent required; associate's or bachelor's degree preferred.
  • Skills and Abilities: Strong communication and interpersonal skills, ability to work independently and as part of a team, high level of tolerance and understanding for individuals with urgent and multiple case management and health needs.
