Mental Health Case Manager II Bilingual Spanish

Telecare Corporation seeks a dedicated and compassionate Case Manager II to join our team. As a primary support and case manager, you will work closely with individuals with mental illness and other co-occurring disorders to help them live independently in their chosen community.
Key Responsibilities:
- Provide direct supervision and support to individuals with mental illness and other co-occurring disorders.
- Assist clients in achieving their goals and living independently in their community.
- Develop and implement individualized treatment plans to meet the unique needs of each client.
- Collaborate with other healthcare professionals to ensure comprehensive care and support.
- Provide crisis intervention and medication support services as needed.
Requirements:
- High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor's degree in Social Services.
- Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
- Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
- Bilingual Spanish.
