Case Manager II

The April Parker Foundation is seeking a highly skilled and compassionate Case Manager II to provide comprehensive case management services to individuals and families affected by violence. This role involves assessing needs, developing intervention plans, coordinating services, and advocating for clients to ensure they receive appropriate support and resources.
The ideal candidate will have a minimum of 5 years of experience in case management or a similar role, with knowledge of trauma-informed care and experience working with individuals impacted by violence. Excellent assessment and problem-solving skills, strong communication and interpersonal skills, and the ability to work effectively with diverse populations are essential.
The Case Manager II will work closely with community partners and stakeholders to implement violence prevention and intervention strategies. They will conduct thorough assessments, provide crisis intervention, and develop safety plans to address immediate and long-term needs. Additionally, the Case Manager II will collaborate with internal and external teams, facilitate support groups, and track progress towards goals.
We offer a competitive salary range of $22.00-$29.00 for Bachelor's Degree holders and $32.21-$34.00 for Master's Degree holders. Our benefits package includes QSEHRA, 401(k) retirement plan, paid time off, and annual shutdown.
