Case Manager I

At Tarzana Treatment Centers, Inc., we are seeking a highly skilled Case Manager I to join our interdisciplinary team. As a Case Manager I, you will play a vital role in providing care coordination services to patients, including linkage to needed resources.
Key Responsibilities:
- Conduct comprehensive assessments of patients' needs and develop individualized treatment plans.
- Provide education and support to patients and their families on substance use disorder treatment options and resources.
- Collaborate with healthcare providers, social services, and other community agencies to ensure seamless care coordination.
- Document patient progress and maintain accurate records.
- Participate in staff meetings and training sessions to stay up-to-date on best practices and program developments.
Requirements:
- Minimum one year of experience in a Behavioral Healthcare setting.
- Enrollment in an academic Addiction Studies program and registration with CCAPP, CAADE, or CADTP.
- Bilingual English/Spanish strongly preferred.
- Valid California driver's license and insurance.
