Job Title: Patient Retention Specialist

Positive Impact Health Centers INC is seeking a highly motivated and detail-oriented Patient Retention Specialist to join our team. As a Patient Retention Specialist, you will play a critical role in ensuring that our patients receive the best possible care and support.

Key Responsibilities:
  • Develop and implement comprehensive patient retention strategies to improve patient engagement and retention rates.
  • Collaborate with healthcare teams to identify and address patient barriers to care, including transportation, housing, and social determinants of health.
  • Provide exceptional customer service to patients, responding to their needs and concerns in a timely and compassionate manner.
  • Work closely with community partners to develop and maintain relationships that support patient retention and engagement.
  • Monitor and analyze patient data to identify trends and areas for improvement.
  • Develop and maintain accurate and up-to-date records of patient interactions and outcomes.
Requirements:
  • Bachelor's degree in a related field, such as healthcare administration or social work.
  • Minimum 2 years of experience in patient retention or a related field.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, healthcare teams, and community partners.
  • Strong analytical and problem-solving skills, with the ability to collect and analyze data to inform patient retention strategies.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
