Role Summary

The Referral Coordinator plays a pivotal role in managing and facilitating specialty care for patients requiring external consultations as directed by their primary care provider. This position is essential in ensuring the seamless progression of all referrals from initiation to the receipt and distribution of specialty reports to the referring provider.

Essential Qualifications

Candidates must demonstrate a professional and respectful demeanor when interacting with both internal and external stakeholders. The ability to juggle multiple priorities and meet deadlines is crucial. Strong analytical and critical thinking abilities are necessary.

  • Experience: A minimum of 2 years in a related field is preferred.
  • Required Skills: Familiarity with medical terminology, proficiency in Microsoft Office, and excellent verbal and written communication skills. Knowledge of local insurance providers and plans is essential.
  • Preferred Skills: Bilingual proficiency in English and Spanish is advantageous.
Educational Background

An Associate's degree in a healthcare-related discipline is required.

Key Responsibilities
  1. Facilitate patient referrals to specialty providers throughout the entire referral lifecycle.
  2. Gather and compile pertinent information regarding the patient's clinical history and referral requirements. Provide necessary clinical details to specialists as per referral protocols.
  3. Engage with specialty offices to arrange appointments.
  4. Communicate with insurance companies to verify prior authorization prerequisites.
  5. Organize and monitor orders, as well as scheduled and pending referrals.
  6. Maintain accurate documentation in the electronic health record to ensure all communications with patients and referral sources are properly recorded.
  7. Discuss referral details and expectations with patients, including any financial responsibilities.
  8. Serve as a resource for patients or providers with inquiries or issues related to referrals, potentially advocating on behalf of the patient with insurance carriers to secure timely approvals for necessary services.
  9. Establish and nurture relationships with designated service providers.
  10. Ensure timely attention to referrals, prioritizing urgent matters as needed.
  11. Collaborate with assigned staff to ensure that consult notes are received and forwarded to the provider for evaluation.
  12. Engage in quality improvement initiatives by monitoring, measuring, analyzing, and enhancing program goals and services.
  13. Support compliance with current incentive, regulatory, and certification standards through appropriate documentation and participation in relevant initiatives.
  14. Perform additional duties as required.
