Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Home Health & Hospice team at Physicians Choice Home Health Inc. As an LPN, you will play a vital role in delivering exceptional patient care and ensuring our patients receive the highest level of service.

Key Responsibilities
  • Verify and execute care plans as prescribed by physicians and under the guidance of a Registered Nurse
  • Provide essential nursing treatments, promptly address patient pain, and assist with daily living activities
  • Administer medications, ensure the availability of necessary supplies, and maintain accurate patient records
  • Liaise with supervising RNs or physicians on patient assessments and any changes in conditions
  • Contribute to team meetings to ensure cohesive patient care coordination
Requirements
  • Current Licensed Practical Nurse (LPN) license in the state of practice
  • Home Health and/or Hospice experience as an LPN preferred
  • Ability to utilize full range of movement to transfer patients
  • Effective verbal and written communication and time management skills
  • Valid Driver's License (clean record) and reliable transportation
  • Proof of current auto insurance
  • Ability to pass Criminal Background Check
  • Current CPR Certification and Negative TB Test
