As a Home Health Nurse, you will have the opportunity to make a profound impact in your community, working collaboratively with our multi-disciplinary team of experts to achieve the heartfelt outcomes and goals of our patients.

Responsibilities

  • Perform comprehensive assessments and develop personalized care plans for patients
  • Collaborate with our team of experts, including PT, OT, ST, MSW, and paraprofessionals, to ensure seamless support for patients
  • Supervise your caseload, nurturing the growth and development of LPNs and Home Health Aides
  • Facilitate care coordination, ensuring that patients receive the highest quality care

Qualifications

  • Active RN License
  • One (1+) Year of Clinical Excellence
  • CPR Certification
  • Valid Driver's License
