Overview

The Economic Services Division (ESD) is a vital part of the Department of Children and Families (DCF), working to provide economic benefits to Vermonters in need. As a key member of our team, you will be responsible for delivering case management services to families, helping them overcome obstacles and achieve their goals.

The Role

As a Reach Up Case Manager II, you will work closely with families to identify their strengths and challenges, and develop personalized plans to help them succeed. You will provide coaching and support to help families navigate the Reach Up program, and work collaboratively with other ESD staff to ensure that families receive the resources they need.

Responsibilities
  • Deliver case management services to families, including assessment, planning, and coordination of services
  • Provide coaching and support to help families achieve their goals
  • Work collaboratively with other ESD staff to ensure that families receive the resources they need
  • Develop and maintain relationships with community partners and stakeholders
  • Participate in ongoing training and professional development to stay up-to-date on best practices and program requirements
Requirements

To be successful in this role, you will need:

  • A bachelor's degree in a human services field, or equivalent experience
  • At least one year of experience in case management or a related field
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
