Job Title: Wellness Liaison

Sun River Health is seeking a highly motivated and compassionate Wellness Liaison to join our team at our Sutphin location. As a key member of our outreach and education team, you will play a vital role in connecting our community members with essential health services.

Job Summary:

The Wellness Liaison will provide outreach and education services to coordinate intake and enrollment into Health Home and all Sun River Health service lines and programmatic services. This role requires strong interpersonal skills to build relationships with our target patient/client base and work collaboratively with other departments and community-based agencies.

Key Responsibilities:
  • Participate in outreach and educational events to promote health services and engage with the community.
  • Serve as a liaison between clients, providers, and account management staff to ensure seamless communication and coordination.
  • Provide outbound reminder calls to clients prior to their scheduled appointments and assist with securing appointments for SRH Health Center services.
  • Conduct Health Home pre-eligibility screenings and intakes for eligible clients and achieve monthly intake targets.
  • Assist with client outreach, education of SRH services, and linkage to Health Center and Health Home.
  • Arrange transportation and assist with health insurance applications as needed.
  • Enter data accurately and timely into required databases, including demographic information, insurance information, assessments, and progress notes.
  • Provide necessary documentation and information to Departmental Health Home Manager in each county/borough.
  • Participate in community events, such as health fairs, and conduct on-street marketing of flyers and service information pamphlets.
  • Support Health Center and Health Home by participating in Kiosk/Mobile events in specified locations to engage the community and conduct Health Home Intakes.
  • Act as an initial service ambassador to promote and educate referral sources, clients, and community members about all SRH services while providing exceptional customer service.
  • Support SRH's mission, values, strategic goals, and high standards of customer service.
  • Adhere to the Corporate Compliance Program, including confidentiality of HIPAA protected health information.
  • Participate in team meetings to ensure effective communication and collaboration.
Requirements:

Minimum Education Requirement: Bachelor's degree preferred, High School Diploma/GED required with an additional four years of relevant experience in customer service in appropriate service industries.

Minimum Work-Related Experience: 2-4 years.

Hourly Rate: $24.00/hour.
