Key Responsibilities:

  • Provide high-quality patient care and education in a collaborative environment.
  • Develop and implement comprehensive care plans tailored to individual patient needs.
  • Work closely with healthcare providers and the integrated care team to ensure seamless patient care.
  • Stay up-to-date with industry developments and best practices in internal medicine and nephrology.

Requirements:

  • Current license to practice as a Nurse Practitioner in the state of California.
  • Current DEA license or eligibility to obtain within 90 days of hire.
  • Current CPR or BLS certification.
  • Reliable personal transportation and a valid driver's license.
  • Minimum of two years' experience as a Nurse Practitioner.
  • Preferred experience in internal medicine, nephrology, or case management.
