About the Opportunity:
We are seeking a dedicated and compassionate Registered Nurse to join our team as a Comprehensive Care Coordinator. This is an exciting opportunity for a professional to make a meaningful impact in the lives of our patients and their families.
Job Summary:
The successful candidate will be responsible for coordinating comprehensive care to patients, including assessing patients' needs, developing individualized plans of care, and ensuring seamless care delivery. This will involve working closely with interdisciplinary teams to promote patient independence and optimal health outcomes.
Key Responsibilities:
- Assessment and Planning: Conduct thorough assessments of patients' health status, environment, and support systems to develop comprehensive plans of care.
- Care Coordination: Collaborate with patients, families, and healthcare teams to ensure timely and effective care delivery.
- Education and Support: Provide education and support to patients and families to promote independence and optimal health outcomes.
- Documentation and Communication: Maintain accurate and detailed records of patient interactions and care activities, communicating effectively with healthcare teams to ensure continuity of care.
Requirements:
- Licensure: Must maintain a current, unrestricted registered nursing license with the Minnesota Board of Nursing that is in good standing.
- Certifications: BLS Certification or ability to obtain within two (2) months.
- Experience: One year RN experience, or six-month's RN experience along with one-year LPN experience in Home Care, TCU or SNF.
- Skills: Strong assessment, critical thinking, and communication skills, with the ability to work independently and as part of a team.
