Position Overview

The Patient Care Coordinator plays a vital role in managing the care of adult and geriatric patients within a Long-Term Acute Care (LTAC) setting. This position is responsible for assessing patient needs and planning care that optimally addresses their health requirements.

Key Responsibilities
  • Utilization Review: Conducts thorough evaluations for medical necessity, ensuring that admissions, continued stays, and discharges align with established criteria.
  • Discharge Planning: Develops and implements tailored discharge plans that cater to the unique needs of patients and their families.
  • Case Management Services: Provides essential case management support to patients and their families, addressing ongoing care, financial concerns, and other needs.
  • Collaboration: Works closely with the interdisciplinary healthcare team to ensure that patient care outcomes are achieved within the appropriate length of stay.
  • Referral Management: Identifies cases requiring physician advisor intervention when care does not meet defined criteria.
  • Regulatory Compliance: Ensures all discharge planning elements meet the standards set by regulatory and accrediting bodies.
  • Patient Care Facilitation: Assists in the execution of physician orders to promote timely patient care throughout their hospital stay.
  • Quality Improvement: Actively participates in identifying opportunities for enhancing patient care and managing patient complaints.
  • Data Collection: Collects and analyzes data for various case management processes to support internal and external benchmarking.
  • Patient Education: Provides education to patients and families regarding available resources and effective use of healthcare services.
  • Hospital Stay Management: Strives to manage patient stays to facilitate timely discharges.
  • Pre-Admission Assessments: Conducts assessments and insurance verifications to ensure medical necessity prior to admission.
Qualifications

Applicants must possess a current nursing license in the State of Alabama. A minimum of three (3) years of nursing experience is required, with at least two (2) years in an acute care hospital environment preferred.

Physical Requirements

Candidates should be able to perform the following physical activities throughout the workday:

  • Walk, sit, stoop, or stand intermittently.
  • Utilize hands and fingers effectively.
  • Maintain adequate sight and hearing, or use prosthetics as necessary.
  • Reach in various directions as needed.
