Job Summary: We are seeking a highly motivated and compassionate Case Manager to join our team at RHA Health Services. As a Case Manager, you will be responsible for providing basic case management services to medically fragile children who are at risk of institutionalization. Your primary goal will be to ensure the health, safety, and well-being of these individuals, while also coordinating services with our RN team to ensure quality care.

Key Responsibilities:
Provide case management services to medically fragile children and their families
Coordinate services with our RN team to ensure quality care
Develop and implement person-centered plans to meet the unique needs of each individual
Collaborate with community resources to provide support and services to individuals and families

Requirements:
Bachelor's degree in social work or a human services field from an accredited institution
1+ year of experience in case management or a related field
Strong communication and interpersonal skills
Ability to work independently and as part of a team
