Job Description

This is a full-time 12-month position as a Population Health Fellow in the Medication Therapy Management Telehealth Center at The University of Findlay. The position is for one year, with a second year option available to qualified candidates upon mutual agreement of sponsoring entities.

Key Responsibilities
  • Operate primarily from the University of Findlay Telehealth Center, providing population health services, with occasional required travel to Mercy Health sites within Ohio and Kentucky.
  • Work on a team responsible for developing, implementing, and analyzing population health strategies, with a focus on medication therapy management.
  • Conduct MTM sessions, including comprehensive medication reviews, personal medication records, and medication-related action plans.
  • Supervise graduate assistants, student employees, and pharmacists, and advise and mentor students on research projects.
  • Develop and refine policies and procedures related to the operation of the MTMTHC, and ensure compliance with CMS billing, fraud waste and abuse, and HIPAA requirements.
  • Provide training for students and pharmacists, and perform other duties as assigned.
Requirements
  • Doctor of Pharmacy degree obtained prior to start date.
  • Licensed, registered pharmacist in the State of Ohio with no restrictions on license.
  • Personal professional liability insurance at all times.
  • Eligibility to serve as a preceptor for pharmacy students in the State of Ohio.
  • Ability to work in and understand the academic environment.
  • Excellent communication and presentation skills.
  • Excellent interpersonal and communication skills, and ability to work well with other pharmacists, health care providers, and students.
Preferred Qualifications
  • APhA Medication Therapy Management Certificate training.
  • Experience with OutcomesMTM/Mirixia MTM platform.
