Population Health Fellow

This is a full-time 12-month position as a Population Health Fellow in the Medication Therapy Management Telehealth Center at The University of Findlay. The position is for one year, with a second year option available to qualified candidates upon mutual agreement of sponsoring entities.
Key Responsibilities- Operate primarily from the University of Findlay Telehealth Center, providing population health services, with occasional required travel to Mercy Health sites within Ohio and Kentucky.
- Work on a team responsible for developing, implementing, and analyzing population health strategies, with a focus on medication therapy management.
- Conduct MTM sessions, including Comprehensive Medication Review, Personal Medication Record, Medication-related Action Plan, intervention, and/or referral, and documentation and follow-up on recommendations.
- Supervise graduate assistants, student employees, and advise and mentor students on research projects.
- Develop and refine policies and procedures related to the operation of the Medication Therapy Management Telehealth Center.
- Ensure compliance with CMS billing, fraud waste and abuse, and HIPAA requirements.
- Doctor of Pharmacy degree obtained prior to start date.
- Licensed, registered pharmacist in the State of Ohio with no restrictions on license.
- Personal professional liability insurance at all times.
- Eligibility to serve as a preceptor for pharmacy students in the State of Ohio.
- Ability to work in and understand the academic environment.
- Skill in program development and execution.
- Demonstrated commitment to the principles of diversity.
- Excellence in communication and presentation skills.
- Excellent interpersonal and communication skills, and ability to work well with other pharmacists, health care providers, and students.
- APhA Medication Therapy Management Certificate training.
- Experience with OutcomesMTM/Mirixia MTM platform.
