Population Health Fellow in Medication Therapy Management

This is a full-time 12-month position as a Population Health Fellow in the Medication Therapy Management Telehealth Center. The position is for one year, with a second year option available to qualified candidates upon mutual agreement of sponsoring entities.
Essential Functions
This position requires regular, predictable, reliable attendance in the worksite environment. Work on a team that is, collectively, responsible to:
- The Fellow will operate primarily from the University of Findlay Telehealth Center, providing population health services, with occasional required travel to Mercy Health (MH) sites within Ohio and Kentucky.
- The majority of the Fellow's time will be spent working directly with the MH population health specialists on projects specific to the institution's needs, with the remainder of the time spent with University of Findlay College of Pharmacy (UFCPHM) faculty to gain an understanding of the context for population health and an expertise in developing, implementing, and analyzing population health strategies. A research project, presentation, and teaching responsibilities are also expected. Candidates should possess excellent communication, supervisory, and organization skills.
- The Fellow will also perform all other duties deemed necessary for this position, such as:
o Conduct MTM Sessions, which includes:
Comprehensive Medication Review (CMR)
Personal Medication Record (PMR)
Medication-related Action Plan (MAP)
Intervention and/or referral
Documentation and follow-up on recommendations, including targeted interventions
o Chronic Care Management and/or Comprehensive Primary Care Plus activities
o Medication Reconciliation and Transition of Care services
- Supervise graduate assistants and student employees.
- Advise and mentor students on research projects.
- Advise, supervise, and mentor IPPE students, APPE students, and residents.
- Create invoices for services, distribute invoices, and ensure payment for services.
- Keep track of student and pharmacist employee hours and ensure they are paid for services.
- Comply with and ensure employees comply with all CMS billing, fraud waste and abuse, and HIPAA requirements.
- Interview and hire student employees and non-faculty pharmacists.
- Conduct performance appraisals for non-faculty pharmacists who are employed in the MTMTHC.
- Reneew the State Board of Ohio Terminal Distributor/Consulting Pharmacy license annually.
- Develop and refine policies and procedures related to the operation of the MTMTHC.
- Ensure that there is access to adequate pharmacy references and that references are updated for student and pharmacist use in the MTMTHC.
- Provide training for students and pharmacists.
- Other duties as may arise from time to time and as may be assigned to the employee.

This person may supervise pharmacy student interns and student workers.
Work Environment
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, cameras, records, monitoring devices, phones, photocopiers, filing cabinets, and fax machines.
Physical Demands
This is largely a sedentary role; however, some filing is required. This would require the ability to lift files, open filing cabinets, and bend or stand as necessary.
Position Type and Expected Hours of Work
This full-time fellowship is for one year as the Population Health Fellow in its Medication Therapy Management Telehealth Center.
Travel
Travel is expected for this position. A valid US Drivers' license with a safe driving record is required.
Required Education and Experience
- Doctor of Pharmacy degree must be obtained prior to start date.
- Must be a licensed, registered pharmacist in the State of Ohio with no restrictions on license.
- Must carry personal professional liability insurance at all times.
- Be eligible to serve as a preceptor for pharmacy students in the State of Ohio.
- Ability to work in and understand the academic environment.

- Excellent interpersonal and communication skills, and ability to work well with other pharmacists, health care providers, and students.
- Possess a personal NPI number.

- Have no pharmacy license restrictions imposed by the Ohio State Board of Pharmacy.
- APhA Medication Therapy Management Certificate training desired.
- Experience with OutcomesMTM/Mirixia MTM platform desired.
