About the Role

Sunrise Senior Living is seeking a dedicated and compassionate Care Manager to join our team. As a Care Manager, you will be responsible for providing high-quality care and services to a consistent group of residents in our assisted living and reminiscence neighborhoods.

Key Responsibilities
  • Build meaningful relationships with residents and their families, providing assistance with activities of daily living and attending to individual care needs.
  • Notify management of changes in condition and recommend adjustments in the level of care and service.
  • Assist residents in life skills and other life-enriching activities as indicated on their individual profile.
  • Participate in the development of Individualized Service Plans (ISPs) and monthly updates.
  • Responsible for a designated group of residents during the shift, ensuring their safety and well-being.
  • Partner with the community team to ensure compliance with OSHA requirements and promotion of Risk Management programs and policies.
  • Respond to the dining needs of residents and guests while maintaining Sunrise hospitality and service standards.
Requirements
  • Dedication to serving seniors with excellent customer service skills.
  • Positive attitude and flexibility to perform various duties in service to residents.
  • High School diploma or equivalent required; certifications may be necessary per state regulations.
  • Must be at least 18 years of age.
  • Previous experience working with seniors preferred.
  • Ability to make choices, decisions, and act in the resident's best interest.
  • Competent in organizational and time management skills.
  • Demonstrate good judgment, problem-solving, and decision-making skills.
