Job Summary

We are seeking a highly skilled Care Manager - Inpatient to join our team at Atlantic Health System. As a key member of our healthcare team, you will be responsible for providing exceptional care and services to our patients across the continuum of illness.

Key Responsibilities
  • Manage and coordinate the care delivered to patients through patient/family assessment and multidisciplinary collaboration.
  • Ensure daily chart review, utilization reviews, and comprehensive clinical documentation in the electronic medical record.
  • Provide interdisciplinary patient care rounds and identify and resolve quality of care issues, barriers to care progression, and discharge delays.
  • Formulate, coordinate, and implement discharge plans and efficient care plans to ensure safe, timely, and effective discharge.
  • Work with the interdisciplinary healthcare team to manage and assist the patient care plan.
  • Perform variance tracking, outcome analysis, and identify problems to assess the effectiveness of clinical pathways.
Requirements
  • Bachelor's degree in Nursing
  • Valid New Jersey Nursing License Required
  • Minimum of 3 years of Acute Care experience is strongly preferred
