Compensation: Starting at $27 per hour (additional shift differential may apply) with potential increases based on experience and qualifications.
Role Overview:
As a key member of our healthcare team, you will provide essential nursing care within an integrated framework that encompasses behavioral health, addiction treatment, and primary care services.
Key Responsibilities:
- Perform thorough assessments of patients' physical and mental health, including substance use history and social factors affecting health.
- Create personalized care plans tailored to each patient's unique needs and treatment objectives, incorporating behavioral health, addiction, and primary care strategies.
- Administer medications and provide medication-assisted treatment (MAT) for substance use disorders, monitoring patient responses closely.
- Collaborate with a multidisciplinary team of healthcare professionals to ensure comprehensive and coordinated patient care.
- Deliver evidence-based counseling and psychoeducation to patients and their families, focusing on mental health, addiction, and chronic disease management.
- Conduct regular follow-up appointments to assess patient progress, offer ongoing support, and modify treatment plans as necessary.
- Maintain precise and current medical records, adhering to regulatory standards and best practices.
- Engage in continuous professional development and educational opportunities.
Qualifications:
- Current nursing license (RN or LPN) in Florida.
- Experience in integrated healthcare settings, particularly in behavioral health and addiction medicine.
- Strong knowledge of mental health disorders and treatment methods.
- Excellent assessment, critical thinking, and communication skills.
- Able to work both independently and as part of a collaborative team.
