Job Title: Licensed Practical Nurse

Nurses and More, Inc. is seeking a skilled Licensed Practical Nurse (LPN) to join our team. As a LPN, you will provide high-quality patient care in various settings, including long-term care, assisted living facilities, and memory care facilities.

Job Responsibilities:
  • Provide skilled nursing care within the nursing scope of practice, the standard of care, and as ordered on the plan of care and in compliance with physician orders.
  • Supervise visits by a Registered Nurse within the patient's home at least every 30 days.
  • Document skilled care and/or services provided in compliance with federal and state laws and regulations, and Agency policies and procedures.
  • Participate in case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, to assure the patient care needs are met.
  • Promote and provide patient/family/caregiver education using various verbal and written communication.
  • Communicate with all members of the health care team, including physicians appropriately and in a timely manner.
Requirements:
  • Current State License Practical Nursing License
  • Current CPR (cannot be online course)
  • Current Driver's License
  • Physical within 1 year (must include state 'free of communicable diseases')
  • Current TB Skin Test (within 12 months), Chest X-ray (must be within 5 years), QuantiFERON (within 12 months)
  • Two Character reference (must be co-workers, not supervisors)
  • Current Alzheimer's Training (must be 2 hours)
  • HIV/AIDS CEU (Required only if you have held your nursing license for 2 years or longer)
  • Proof of full COVID vaccination (per CMS vaccine mandate) OR submit an approved medical exemption (with supportive documentation) OR religious exemption prior to hire date
