Position Overview

Key Responsibilities:
  • Program Oversight: Assume full accountability for the programmatic elements associated with your caseload, ensuring adherence to state and local guidelines.
  • Leadership: Offer guidance and support to Direct Support Professionals (DSP), stepping in to provide direct assistance as necessary.
  • On-Call Support: Maintain availability for on-call supervision to address any immediate needs of individuals receiving services.
Specific Duties:
  • Plan Development: Collaborate with the Coordinator of Community Service (CCS) to create, implement, and assess the progress of individualized person-centered plans (PCP).
  • Documentation Management: Ensure the CCS is equipped with all required documentation to finalize the PCP efficiently.
  • Timely Finalization: Confirm that all PCPs are completed promptly to avoid billing delays, ensuring they encompass sufficient units for safe operational levels.
  • Regulatory Compliance: Guarantee that all services align with federal and state waiver standards as well as organizational expectations.
  • Site Visits: Conduct weekly visits to residential and supported living facilities to maintain oversight and connection with service recipients.
  • Crisis Intervention: Provide immediate support to individuals experiencing crises.
  • Assessment Oversight: Ensure all necessary assessments are conducted as required by the team.
  • Record Maintenance: Keep current and accurate records in the main files of residents.
  • Daily Operations: Supervise daily errands such as shopping and banking, ensuring proper management of individual budgets.
  • Financial Oversight: Collect receipts weekly and assist individuals in managing their finances as needed.
  • Home Inspections: Perform monthly evaluations of living environments and coordinate necessary repairs.
  • Service Coordination: Oversee job schedules to ensure all services are delivered effectively while adhering to budget constraints.
  • Schedule Management: Create and distribute schedules to individuals and DSPs, highlighting important dates and appointments.
  • Documentation Monitoring: Ensure completion of required documentation, including communication logs and incident reports, to maintain compliance.
  • Training Compliance: Verify that employees meet state and agency training requirements, participating in necessary training sessions.
  • Staff Development: Train DSPs to empower individuals in taking on responsibilities associated with independent living.
  • Advocacy Training: Educate DSPs on encouraging self-advocacy among individuals, assisting them in exercising their rights.
  • Additional Duties: Perform any other responsibilities as assigned by the agency.
Qualifications:

Minimum Requirements: A Bachelor’s degree from an accredited institution or a minimum of three years of experience in the human services sector, including at least one year in a community residential program.
