Job Summary:The ideal candidate will have a strong clinical background, excellent communication skills, and the ability to work autonomously. You will be responsible for providing nursing care and supportive services in accordance with the physician/provider treatment plan and the nursing process of assessment, diagnosis, planning, implementation, and evaluation.
Responsibilities:- Provide nursing care and supportive services to patients in their homes
- Assess patient needs and develop individualized care plans
- Implement and evaluate treatment plans in collaboration with the healthcare team
- Communicate effectively with patients, families, and healthcare professionals
- Manage scheduling visits, documentation time, and meetings effectively
- Licensed Vocational Nurse License in California
- Preferred 1-2 years of professional experience in home health and hospice
- Current CPR Certification
- IV Certification
- Wound Care and/or wound vac experience preferred
- Ability to drive within 30-60 miles a day
- Ability to pass drug and background screenings
This position requires the ability to work in a fast-paced, dynamic environment with a focus on providing high-quality patient care. You will be working in a community-based setting, providing care to patients in their homes.
