About the Role:
As a Nurse Practitioner specializing in Physiatry, you will collaborate closely with our post-acute rehabilitation team to enhance patient recovery and promote autonomy. This role is dedicated to providing care within a designated skilled nursing facility (SNF) or post-acute rehabilitation center.

Position Details:
This is a full-time, 40-hour W-2 position located in a skilled nursing facility.

Key Responsibilities:
- Deliver specialized Physiatry/PM&R services as a rehabilitation consultant.
- Exhibit strong clinical leadership and collaboration within your facility.
- Manage common conditions encountered in SNFs, including CVA, progressive neurological disorders, and postoperative orthopedic cases.
- Work in conjunction with Physical Therapy (PT), Occupational Therapy (OT), and Speech-Language Pathology (SLP) teams to enhance patient functionality and quality of life.
- Monitor skilled therapy patients to assess their progress.
- Identify and address barriers to therapy, such as pain management and neurogenic conditions.
- Participate in multidisciplinary meetings to foster teamwork and communication.
- Engage in ongoing education and culture-building activities, including lectures and regional networking events.

Qualifications:
- Nurse Practitioner/APRN/Physician Assistant credentials.
- Active Connecticut License.
