Role Overview:

As a Nurse Practitioner specializing in Physiatry, you will collaborate closely with our post-acute therapy team to enhance rehabilitative outcomes and support patient autonomy. This role focuses on delivering care within a designated skilled nursing facility (SNF) or post-acute rehabilitation center.

Position Details:

This is a full-time, 40-hour W-2 position.

Key Responsibilities:

  • Deliver specialized Physiatry/PM&R services as a rehabilitation consultant.
  • Exhibit strong clinical leadership and collaboration within your facility.
  • Manage common conditions in SNFs, including CVA, progressive neurological disorders, and postoperative orthopedic cases.
  • Collaborate with Physical Therapy (PT), Occupational Therapy (OT), and Speech-Language Pathology (SLP) teams to enhance patient function and quality of life.
  • Monitor therapy patients to assess their progress.
  • Identify and address barriers to therapy, such as pain management and neurogenic conditions.
  • Participate in multidisciplinary meetings to integrate therapy and internal medicine.
  • Engage in ongoing education and culture-building initiatives, including lectures and regional gatherings.

Qualifications:

  • Nurse Practitioner/APRN/Physician Assistant credentials.
  • Valid Connecticut License.
  • Eligibility to work in the U.S.
