POSITION OVERVIEW

The Nurse Practitioner (NP) in Primary Care is a certified, licensed healthcare professional who delivers direct patient care and holds delegated prescriptive authority. This role is recognized as an independent billing provider within the healthcare framework, adhering to the regulations set forth by Medicare and Medicaid. The Advanced Practice Provider offers medical services based on the defined scope of practice established by the supervising physician.

Typically, these practitioners cater to patients in settings such as internal medicine, family medicine, geriatrics, or pediatrics.

CORE RESPONSIBILITIES

  • Conducts thorough assessments of patients' physical and mental health through comprehensive or focused examinations and medical histories.
  • Orders and interprets laboratory tests and diagnostic evaluations to identify patient health issues and needs.
  • Prescribes and manages medication therapies and other therapeutic interventions in line with established protocols, prioritizing efficacy, safety, and cost-effectiveness.
  • Advises on lifestyle modifications to mitigate health risks.
  • Develops and executes comprehensive treatment plans based on patient evaluations, evidence-based practices, and in collaboration with physicians and other healthcare team members. Monitors and assesses patient responses to care.
  • Provides guidance to patients and their families regarding medical procedures, health management, medication, nutrition, and wellness strategies. Informs patients about medication regimens and potential side effects or interactions.
  • Keeps detailed records of patient treatment plans and outcomes. Initiates referrals when issues exceed the scope of practice.
  • Offers specialized consultation services related to the provider's area of expertise.
  • Performs procedures within the scope of practice as delegated by the supervising physician.
  • May manage a patient panel in a collaborative model focusing on both acute and chronic conditions, along with population health initiatives.

KEY COMPETENCIES

  • Understanding of medical knowledge and techniques necessary for diagnosing and treating injuries, diseases, and disorders, including treatment options and preventive measures.
  • Familiarity with diagnostic and treatment practices for physical and mental health conditions.
  • Effective verbal and written communication skills.
  • Strong interpersonal abilities.
  • Critical thinking and advanced problem-solving skills.
  • Proficiency in computer systems and relevant software.
  • Capability to create and implement comprehensive patient treatment plans.
  • Aptitude for counseling patients on medical and psychological conditions and treatment strategies.
  • Ability to interpret laboratory and diagnostic test results.
  • Possession of a valid Texas RN license or a compact privilege RN license and Texas APRN certification.
  • Current Nurse Practitioner license in Texas.
  • Basic Life Support certification within 30 days of employment.
  • National Nurse Practitioner certification in the relevant specialty area.

  • EXPERIENCE - Minimum of 1 year in a relevant role
