About the Role

We are seeking a skilled Registered Nurse to join our Admissions team at Heartland Hospice. As a key member of our team, you will be responsible for explaining our services to patients and families, assessing their needs, and admitting them to our hospice care program.

Key Responsibilities
  • Conduct patient assessments to determine eligibility for hospice care
  • Explain our services and benefits to patients and families
  • Collaborate with healthcare teams to ensure seamless transitions
  • Participate in on-call rotations to provide 24/7 support
  • Document patient information and progress in our electronic medical records system
Requirements
  • Current Registered Nurse license in the state where the program is located
  • Bachelor's Degree in Nursing or related field
  • Minimum of 1 year general nursing experience, with hospice experience preferred
  • CPR Certification
