Job OverviewWe are seeking a skilled Memory Care Behavioral Health Specialist to join our team. As a core member of our collaborative care team, you will work closely with patients, their families, and other healthcare providers to deliver high-quality care and support.
Responsibilities- Develop and implement individualized care plans to address the unique needs of patients with dementia and mental health conditions.
- Provide education and support to patients' care teams on Alzheimer's disease, dementia, and mental health diagnoses.
- Establish and maintain effective relationships with patients, families, and healthcare providers to ensure seamless care coordination.
- Collaborate with psychiatric consultants to develop and implement treatment plans.
- Facilitate referrals for clinically indicated services outside of the organization.
- Act as a Bluestone ambassador for community staff through education and relationship building.
- Formal education or specialized training in behavioral health, including social work, nursing, psychology, gerontology, or related fields.
- One or more years of experience in memory care and/or dementia-related care.
- Valid driver's license required.
- A competitive salary range of $55,000 - $65,000, commensurate with experience.
