Job Summary

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Clinical Care Specialist. In this role, you will conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.

Responsibilities

  • Conduct home visits to assess the health and well-being of at-risk individuals
  • Develop and implement personalized care plans to address the unique needs of each patient
  • Collaborate with physicians, case managers, social workers, and other healthcare professionals to ensure comprehensive care
  • Provide education and support to patients and their families to promote healthy behaviors and disease management
  • Document all interactions and care provided in accordance with Matrix protocols

Requirements

  • Master's degree in Nursing or Physician Assistant studies
  • Current RN and NP or PA licensure in the state of practice
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
  • Strong communication and interpersonal skills
  • Ability to travel to patient homes and other locations as needed
