Nurse Care Coordinator

About the Role:
We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Avamere Rehab of Rogue Valley. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and contributing to a positive and supportive work environment.
Responsibilities:
- Complete accurate and timely record keeping in regards to patient admissions, transfers, and discharges.
- Coordinate with nursing staff and support personnel to plan daily services, programs, and activities that meet patient needs.
- Prepare and administer medications, reviewing medication cards for completeness and accuracy.
- Review patient care plans to ensure they align with patient goals, problems, and approaches.
- Develop and implement programs, goals, and policies that promote quality patient care.
- Provide leadership and support to nursing personnel, making daily rounds to ensure staff is providing direct care in accordance with patient care plans.
- Develop and maintain positive relationships with nursing staff, other departments, patients, and families.
Qualifications:
- Current, unencumbered LPN license in this state.
- Nursing degree from an accredited college or university.
- Knowledge of nursing and medical practices, laws, and regulations.
- Experience with Electronic Medical Records and computer documentation systems.
- Maintain confidentiality of patient care information in accordance with HIPAA guidelines.
