We are seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to join our team at ConcertoCare. As a member of our value-based healthcare delivery model, you will have the opportunity to provide high-quality, patient-centered care to seniors and older adults with complex care needs in the comfort of their own homes.

ConcertoCare is a tech-enabled in-home care provider dedicated to redefining care and aging for millions of US seniors and other adults with complex care needs. Our mission is to deliver a more holistic, equitable, and compassionate approach to health and wellness.

The Role:

  • Provide comprehensive care, including urgent care and chronic condition management, that aligns with our value-based care delivery model.
  • Collaborate with multidisciplinary teams to cooperatively deliver transitions of care support, social determinants of health interventions, behavioral health evaluation and management, and complex medication management.
  • Deliver exceptional clinical care both in-person and virtually, with in-person settings including private home, congregate living facility, ambulatory clinic, and/or nursing facility.

Requirements:

  • Nurse Practitioner or Physician Assistant with graduation from an accredited health professions school and active national board certification.
  • Active licensure in Tennessee.
  • Active DEA licensure and state-specific controlled substance registration, as applicable.

Salary and Benefits:

We offer a competitive salary range of $120,000 to $135,000, plus annual bonus opportunities. Our total rewards package includes full healthcare coverage, a 401K with match, and a broad range of other health, wellness, and financial benefits.
