About the Role

We are seeking a highly motivated and compassionate Community Health Worker to join our team at UnitedHealthcare. As a Community Health Worker, you will play a vital role in improving the health and well-being of our members by providing personalized care and support.

Key Responsibilities
  • Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care.
  • Utilize both company and community-based resources to establish a safe and effective case management plan for members.
  • Collaborate with patient, family, and healthcare providers to ensure comprehensive care.
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
  • Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members.
  • Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the healthcare team.
  • Document all member assessments, care plan, and referrals provided.
Requirements
  • Bachelor's Degree (or higher) in health-related field/and or social work or Active licensed practical nurse (LPN) license.
  • 1+ years of field-based experience.
  • 1+ years of experience of knowledge of the resources available, culture, and values in the community.
  • Intermediate level of computer proficiency (including Microsoft Outlook, Teams) and be able to use multiple web applications.
  • Valid Driver's license, current automobile insurance with access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area.
  • Must reside within a commutable distance to Winchester, Front Royal, and surrounding communities VA.
  • Ability to travel locally, up to 100 miles round trip and up to 50% of the time.
  • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information and live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.
Preferred Qualifications
  • Community Health Worker (CHW) Accreditation.
  • Experience working in Managed Care.
  • Knowledge of Medicaid/Medicare population.

  • Ability to work independently and maintain good judgment and accountability.
  • Demonstrated ability to work well with others.
  • Strong organizational and time management skills.
  • Ability to multi-task and prioritize tasks to meet all deadlines.

  • Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying information in a manner that others can understand, as well as ability to understand and interpret information from others.
