About the Role

At UnitedHealth Group, we are seeking a highly skilled Behavioral Health Care Advocate to join our team. As a key member of our organization, you will play a critical role in improving health outcomes for our members by providing exceptional care and support.

Key Responsibilities
  • Visit members in their homes and other settings to provide personalized care and support.
  • Develop and implement comprehensive care plans to address the unique needs of each member.
  • Collaborate with healthcare providers and community resources to ensure seamless care coordination.
  • Provide education and support to members on condition management, medication adherence, and healthy lifestyle choices.
Requirements
  • Licensed Master's degree in Psychology, Social Work, Counseling, or Marriage and Family Counseling; or Licensed Ph.D.
  • Active, unrestricted, independent license in Virginia: LCSW, LMFT, LPC, or LP
  • 1+ years of experience working in behavioral health
  • Proficient computer skills and good working knowledge of Microsoft Word and Outlook
  • Access to reliable transportation and secure, high-speed internet
Preferred Qualifications
  • 1+ years of experience working with Medicaid population
  • 1+ years of Community Mental Health experience
  • 1+ years of experience working with local and state community resources in Virginia
  • Medical / Behavioral setting experience
  • Dual diagnosis experience with mental health and substance abuse
