Job Summary:

Trinity Health is seeking a Per Diem Behavioral Health Counselor II to join our team at SPARC Second Ave. As a member of our multidisciplinary team, you will provide behavioral health services to patients/residents in our facility, assisting with assessment, planning, and implementation of approved plans of patient care.

Responsibilities:

• Obtain and interpret information about patients and their needs.
• Conduct patient education sessions, as assigned.
• Observe and record significant changes, reporting them to appropriate personnel immediately.
• Conduct psychosocial evaluations and gather information to support current DSM diagnoses, as assigned.
• Assist in screening and/or orientation of new patients to the program and program expectations.
• Develop treatment/service plans for assigned residents in a timely manner.
• Assist with discharge planning.
• Plan appropriate activities for assigned patients/residents with the team.
• Include patients/residents, family, and other healthcare professionals in the formulation of plans.
• Administer care consistent with established treatment/service plans.
• Conduct patient education groups, as assigned.
• Provide Group Therapy and/or didactic group sessions, as needed.

Requirements:

• HS diploma or GED with CASAC-trainee required.
• Associates degree with CASAC-t preferred.
• Strong writing and communication skills are required, as well as knowledge of working with community agencies and managed care representatives is desired.
• Experience working with a diverse population and a strong understanding of multicultural issues is preferred.
