RN Home Care Specialist

About the Role:
The Primary Nurse - Home Care is a vital member of our interdisciplinary team, responsible for delivering high-quality patient care in the comfort of our patients' homes. As a skilled and compassionate RN, you will oversee the general oversight, assessment, planning, and construction of the nursing care delivered to our participants. Your expertise will ensure that interventions are carried out in accordance with the participant's care plan and the orders of their medical providers.
Key Responsibilities:
* Assess patients through obtaining history, performing appropriate tests, making or collecting observations from the participant and caregivers, and consulting with other members of the IDT.
* Prepare and present patients for rapid provider review and treatment.
* Arrange for or directly implement care plan interventions and testing, treatment, or other provider orders for a panel of participants.
* Oversee delegated tasks to ensure completion, and monitor outcomes, and make adjustments to care plan as needed.
* Safely and effectively administer medications, tests, and treatments as ordered. Manage the refills of all patient medications with appropriate authorization by the provider.
Requirements:
* Baccalaureate degree in nursing preferred.
* Current Massachusetts license in nursing.
* Current BLS certification required.
* Minimum of 2 years experience in ambulatory, home health, hospital, or sub-acute setting preferred, equivalent training or related experience considered.
* A minimum of one year experience working with a frail or elderly population.
