Job Summary

Partners HealthCare at Home is seeking an Advanced Practice Clinician to join our Home Hospital team. This is a part-time, 24-hour per week role that will involve providing acute-level care to patients in their homes. The ideal candidate will be a patient-centered, proactive, and flexible clinician with a strong ability to work autonomously and as part of a multidisciplinary team.

Job Description

As an Advanced Practice Clinician, you will be responsible for evaluating and managing patients in their homes, working collaboratively with a multidisciplinary team to provide high-quality, patient-centered care. Your duties will include:

  • Providing acute-level care to patients in their homes, including receiving patients at their residence, establishing a treatment plan, and following Home Hospital protocols.
  • Working with the Home Hospital team to identify patients who qualify for admission to the Home Hospital service and completing the admission process.
  • Establishing relationships with inpatient teams at all five MGB sites to foster seamless transitions of care to the Home Hospital service line.
  • Partnering with remote overnight HH Nocturnist APPs to provide cross-coverage for all patients on the HH Service.
  • Collaborating with multidisciplinary Home Hospital team members and patients' longitudinal care team members to ensure safe, effective, equitable, and timely care delivery for Home Hospital patients.
Qualifications

To be successful in this role, you must have:

  • At least 3 years of experience as a Nurse Practitioner (NP) or Physician Assistant (PA), with a minimum of 2 years of Acute Care Inpatient &/or Emergency experience.
  • Current Massachusetts licensure as a NP or PA with a Master's Degree.
  • Advanced Cardiac Life Support (ACLS) certification.
  • Ability to work in a fast-paced, evolving clinical environment with autonomy.
  • Motivated, collaborative team player with excellent oral and written communication skills.
Estimated Salary

The estimated annual salary for this role is $120,000 - $150,000, depending on experience and qualifications.
