About the Opportunity



 



The East Boston Neighborhood Health Center is seeking a highly skilled Registered Nurse (RN) to fill the role of RN Care Manager - HIV Clinical Care Specialist. This is an exciting opportunity for a dedicated and compassionate nurse to make a meaningful impact in the lives of individuals living with HIV/AIDS.



Responsibilities




  • Demonstrate expertise in HIV-specific nursing care and provide comprehensive clinical follow-up to patients, addressing care gaps and promoting adherence to treatment plans.
  • Collaborate with the Project SHINE clinical care management team to develop individualized patient care plans, including adherence and outreach strategies to promote patient engagement.
  • Assume a leadership role on the multi-disciplinary care team, promoting a sense of teamwork and problem-solving.
  • Provide education and support to patients living with HIV on topics related to chronic disease management, adherence, STI screening, and safe sex.
  • Work effectively with patients and other staff members, maintaining a positive attitude and using communication devices appropriately.
  • Demonstrate knowledge of substance abuse conditions, major mental illness, and multiple diagnosed conditions, and set appropriate limits with patients as needed.
  • Implement HIV nurse-on-call procedures, including functioning as the liaison to inpatient facilities and the on-call physician and mental health administrator.
  • Administer medications, tests, and treatments as ordered, and manage patient medication refills with appropriate authorization.
  • Document patient histories, assessments, plans, and treatments thoroughly and accurately.
  • Participate in grant writing and grant reporting activities related to the nursing function, and contribute to quality improvement initiatives.
  • Perform other duties as assigned, including working with the Counseling and Testing team to perform intake STI assessments and risk assessments.


Requirements




  • High School Diploma required.
  • Completion of vocational/technical training beyond high school in an Accredited Nursing Program required.
  • Licensure in Massachusetts as an R.N. or L.P.N. required.
  • BCLS certification (per departmental policy) required.
  • Two years experience in HIV nursing preferred, preferably in a community health setting.
  • Previous experience working with substance abuse and/or HIV Care patients.
  • Spanish Bilingual/Bicultural skills preferred.
  • Excellent interpersonal skills.
  • Proficient in Microsoft Word.
